Seniors Community Support Programs

Wendat Seniors Community Support Programs:

Serving residents of Penetang  (some programs include residents of Midland area)

The Seniors Community Support Programs provides service to those older adults identified as having care needs requiring support to live independently in the community. This program offers access to a basket of services based on assessed need to eligible older adults living in Penetang and in some cases the immediate surrounding area. We strive to impact positively on the wellness, independence and quality of life for at-risk seniors so that they are able to live longer, in better health and in their chosen environment.

Assisted Living Service:

Due to limited resources and capacity issues, we are restricting access to Assisted Living Services to persons residing at the Seniors’ Residence at 44 Dufferin Street, Penetang.  Referrals of community candidates for Assisted Living support will be considered when additional resources are secured.

 
Eligible seniors for this Program may have access to one or more of the following services for the purpose of maintaining independence in their chosen living environment:
  • · Assistance to arrange transportation to medical appointments
  • · Transportation to shopping for necessities
  • · Support with medication & chronic disease management
  • · Support with housekeeping, laundry and minor upkeep
  • · Minor assistance with activities of daily living
  • · Social stimulation & re-integration into   community
  • · Facilitating connection with cultural groups and organizations
  • · 24/7 on-call assistance
Eligible frail seniors living in their own home  environment may have access to the following additional services:
  • · Telephone Safety Checks
  • · Assistance with meal planning & preparation

Our Team:

Our team is composed of a full time Program Supervisor who is also a Registered Practical Nurse and full and part-time Community Support Workers.

Eligibility Criteria:

Seniors Living in Penetanguishene in their own home or living at the Wendat Seniors Residence

  • Aged 65 + with an emphasis on those who are 75+
  • At risk for falls including orthopedic trauma
  • Experiencing difficulties with managing day to day activities
  • Dealing with the management of some chronic disease processes
  • Experiencing some difficulties with cognition as a result of the aging process
  • Isolated socially or environmentally
  • In need of personal contact and minor assistance with activities of daily living
  • Formalized assessment by Wendat’s Program Supervisor indicating eligibility

 

Hours of service:

Business hours: 8:30 a.m. to 5:00 p.m. Monday to Friday

On-duty hours: 8:30 a.m. to 8:30 p.m. 7 days/week with flexibility

24/7 on call coverage

Day Program:

This program offers social and recreation activities in group settings for seniors who would benefit from a wellness oriented program which includes assessment and some health monitoring.

Our Team:

Our team is composed of a full time Program Supervisior who is also a Registered Practical Nurse and full and part-time Community Support Workers, plus a driver.

Eligibility Criteria:

  • Seniors 65 + years living alone in the community or with a caregiver who would benefit from a wellness oriented program which includes assessment and monitoring. (Emphasis is on those seniors 75+)
  • Resides in the Midland/Penetang or immediate outlying area
  • May be experiencing difficulties with day-to-day living
  • Socially or geographically isolated
  • Needing some assistance/support to reintegrate into social activation

Hours of service:

  • Business hours are from 8:30 a.m. – 5:00 p.m. Monday to Friday
  • Program activity hours: 10 a.m. - 4 p.m. Monday, Tuesday, Thursday and FridaysHow to apply:

Social and Congregate Dining Program:

Our in-house cook prepares nutritious meals at very low cost to eligible seniors who wish to share a meal in the stimulation of a social group environment and to those seniors who participate in the Day Program.

Our Team:

Our team is composed of a full time Program Supervisor who is also a Registered Practical Nurse and full and part-time Community Support Workers and a cook.

 

Eligibility Criteria:

  • Seniors 65 + years living alone in the community or with a caregiver who would benefit from experiencing a meal with others, improved nutritional intake, in a wellness oriented program. (Emphasis is on those seniors 75+)
  • Resides in the Midland/Penetang or immediate outlying area
  • May be experiencing difficulties with day-to-day living
  • Socially or geographically isolated
  • Needing some assistance/support to reintegrate into social activation

Hours of service:

  • Business hours are from 8:30 a.m. – 5:00 p.m. Monday to Friday
  • Dining hours: 11:30 a.m. - 1 p.m. Monday, Tuesday, Thursday and Fridays
